Any agency or organization in the target area of Hind County, Mississippi that provides services to those infected or affected by HIV/AIDS, STDs TB and substance abuse.

  • Complete all forms in the packet: Service Providers Membership Application, Memorandum of Agreement, Confidentiality Form, Role and Responsibility Form, Cultural Competency Agreement.
  • Mail these completed forms to: Mississippi Urban Research Center, Jackson State University, P.O. Box 17309, Jackson, MS 39217 or bring them to our main office at the above location.

What do you need to know?
After you have returned a completed Registration Packet, the network management specialist will schedule a meeting with your agency or organization's director to discuss any questions or concerns you may have.
