Any agency or organization in the target area of Hind County, Mississippi
that provides services to those infected or affected by HIV/AIDS, STDs
TB and substance abuse.

- Complete all forms
in the packet: Service Providers Membership Application, Memorandum
of Agreement, Confidentiality Form, Role and Responsibility Form, Cultural
Competency Agreement.
- Mail these completed
forms to: Mississippi Urban Research Center, Jackson State University,
P.O. Box 17309, Jackson, MS 39217 or bring them to our main office at
the above location.

After you
have returned a completed Registration Packet, the network management
specialist will schedule a meeting with your agency or organization's
director to discuss any questions or concerns you may have.
